The diagnostic pattern that catches early CO. Test by leaving the rig — if the headache lifts within 15-30 minutes, suspect CO and do not return until the source is identified.
Multiple people in the same space with similar symptoms
CO does not respect age, sex, or fitness. If two or more occupants share dizziness or nausea simultaneously, suspect CO before suspecting food poisoning.
Continuous CO alarm (not chirp) inside the rig
UL 2034 listed CO alarms sound a 4-beep cadence when CO is detected. A 30-60 second single chirp is a low-battery warning. Never silence a continuous alarm — open the rig and leave.
Generator exhaust pointed at, or wind blowing toward, the rig
CDC investigation reports of fatal cases consistently describe the generator placed 'just outside' — under a slide, beside an open window, or with downwind flow into a vent. Distance alone is not protective; wind direction is.
Pet collapse before human symptoms
Small dogs and cats reach toxic carboxyhemoglobin levels faster than humans because of their higher metabolic rate. A suddenly unresponsive pet near a running generator is a CO event until proven otherwise.
Reddish or cherry-pink skin tone in an unconscious victim
A late-stage clinical sign — CO-bound hemoglobin imparts a characteristic color. By the time skin color is visibly altered, the patient is critically poisoned.

1. Evacuate first, troubleshoot never
Get everyone — including pets — out of the rig and 100+ feet upwind. Do not stop to grab phones, wallets, or shut off appliances. CO impairs judgement; trying to think your way out is the failure mode.

2. Shut the generator only if you can reach the kill switch without re-entering CO
If the generator is external and reachable from outside, push its stop switch. If shutting it down requires going back into the rig — leave it running and wait for fire department.

3. Call 911 with the count of exposed people
EMS dispatches multiple units for multi-patient CO events. Tell dispatch (a) suspected CO, (b) number of people exposed, (c) source (running generator), (d) anyone unconscious.

4. Do not re-enter until cleared with a meter
Fire departments carry calibrated CO meters. Re-entry decision is theirs. CO remains in soft furnishings and clears slowly even after ventilation; an unmetered rig is not safe.

5. Every exposed person goes to the ER, even asymptomatic ones
Carboxyhemoglobin level measurement requires a blood draw. Delayed neurological symptoms (memory deficits, headache, mood changes) can develop 2-40 days post-exposure. ER evaluation establishes the exposure baseline for follow-up.

6. Document for insurance and for the generator manufacturer
If the CO event involved an onboard generator (Onan, Cummins, Generac), the manufacturer wants to know — exhaust failures are often subject to TSBs or open recalls. NHTSA-recall lookup covers motorhome chassis; the generator OEM tracks its own bulletins.
Prevention
Working CO alarm inside the cabin — tested monthly, replaced at 5-7 years
NFPA 1192 §5.6 requires every RV sold for sleeping occupancy to have a listed CO alarm in addition to the smoke alarm. UL 2034 lists the test cycle (press the test button monthly; replace the sensor head per manufacturer interval — typically 5-7 years, occasionally 10 for newer combo units). RV CO alarms are a different listing than residential and live in a high-vibration environment; do not skip the replacement date.
Onboard generator: never run with exhaust blocked or with rig parked against wall / RV-skirt
Onan and similar built-in motorhome generators route exhaust through a tailpipe extending beyond the body. If the rig is winter-skirted, parked against a building, or has a slide cover blocking the tailpipe outlet, exhaust pressure backs up into the engine bay and into the rig. Confirm tailpipe is clear at every setup.
Portable generator: minimum 20 feet from rig, exhaust pointed AWAY, downwind
CDC guidance (built on CPSC case reviews) is a minimum 20-foot setback from any structure for portable generators, with the exhaust outlet pointed away from doors, windows, and air intakes. 20 feet is the floor — if wind is blowing toward the rig, move the generator or move the rig. New CPSC standards (UL 2201 and PGMA G300) require interlocked CO shutoff sensors on portable generators sold from 2020 onward; older generators do not have this.
Annual professional service of onboard generator
Oil change, air filter, spark arrestor inspection (USFS requires functioning spark arrestors on portable generators used on federal land), exhaust manifold and tailpipe leak check. Onan publishes service intervals by model; an RVDA / RVIA-certified shop is qualified to perform the service.
Cold-start CO is higher than warm-running CO
Internal combustion engines emit substantially more CO in the first minutes of cold operation, before the catalyst (if equipped) reaches light-off temperature. Start the generator and let it warm OUTSIDE the rig before bringing AC compressors and microwaves online. Do not start a generator inside any enclosed space, even briefly, ever.
Plan for power-outage events — most CO deaths cluster during storms
CDC and CPSC post-event reports of hurricanes (Laura, Ida, Helene), winter storms, and prolonged outages show repeated patterns of CO fatalities from generators used to power a damaged home. The same logic applies to extended boondocking. Plan generator placement BEFORE the outage starts and confirm CO alarms function.

The U.S. Consumer Product Safety Commission's annual reports on non-fire carbon-monoxide deaths associated with consumer products track an average of approximately 85 deaths per year from portable generators in the United States, with year-over-year peaks aligned with major outage events. Lesson: Generator-related CO deaths concentrate in non-fire poisoning scenarios — running indoors, in garages, in basements, or close enough to a habitable space that wind brings exhaust inside. The countermeasure is distance, wind direction, and a working CO alarm inside the sleeping space.

NFPA 1192 §5.6 requires every recreational vehicle sold for sleeping use to be equipped with a listed CO alarm; UL 2034 governs the listing test. RVIA manufacturer certification, displayed at the entry door, indicates self-certified compliance at build. Field surveys repeatedly find rigs with missing, expired, or disconnected CO alarms, particularly older used units where the alarm sensor head reached end-of-life and was not replaced.
Lesson: Replace the CO alarm unit at the manufacturer's specified end-of-life date, not just the battery. Sensor chemistry degrades and an expired alarm may no longer sound at low CO levels.

Wind direction matters more than distance
CDC investigations of fatal RV-CO cases repeatedly describe generators placed 'twenty feet away' — but downwind, with the exhaust blowing toward an open window or roof vent. The 20-foot rule is a floor. Place the generator so the prevailing wind carries exhaust AWAY from the rig regardless of the absolute distance.
New 2020+ portable generators have CO-sensor auto-shutoff; older ones do not
PGMA G300 and UL 2201 standards (adopted into many state and federal acquisitions) require new portable generators to include a CO sensor that shuts the unit down when ambient CO exceeds 150 ppm (or equivalent time-weighted thresholds). Generators built before these standards have NONE of this protection — your rig's CO alarm is the only line of defense.
Onboard motorhome generator exhaust can fail at the muffler seam or tailpipe joint
Vibration-driven exhaust failures release CO into the basement compartment and from there into the cabin floor. Annual inspection of the muffler, tailpipe, and connection clamps is part of NFPA-aligned RV service. A soapy-water test on a running generator (qualified tech only) reveals exhaust leaks as bubble streams.
USFS / BLM camping requires spark-arrestor-equipped generators
U.S. Forest Service and Bureau of Land Management regulations prohibit operation of internal combustion engines without functioning spark arrestors in fire-restricted areas. The arrestor inspection is part of generator service; an arrestor that has been removed for noise reduction is both a fire risk and a regulatory violation.
Onan EC-30 / EC-50 transfer-switch failures can create back-feed shock hazards
If the onboard generator's automatic transfer switch fails in the wrong position, both shore power and generator output can be live at the same source — a back-feed hazard to anyone working at the pedestal. Trip the rig main before connecting to a new pedestal if you have any reason to doubt transfer-switch health.

Frequently asked questions
How far away should I put my portable generator from the rig?
CDC and CPSC guidance is a minimum 20 feet of separation from any structure, with the exhaust outlet pointed away from doors, windows, and intakes. 20 feet is the floor — adjust based on wind direction. If wind is blowing exhaust toward the rig, move the generator further or reposition.
Is it safe to run the generator overnight while we sleep?
Only if (a) it is an onboard generator with intact exhaust, (b) the rig's CO alarm is tested and working, (c) the exhaust outlet is unobstructed. Portable generators should not be run unattended overnight near a sleeping rig — CDC reviews of fatal cases repeatedly find overnight unattended operation as a factor.
Does opening a window dilute CO from a nearby generator?
No. Open windows can actually draw exhaust inside if the wind cooperates. CO is roughly the same density as air and follows airflow. The correct intervention is distance, wind direction, and a working CO alarm — not ventilation.
My CO alarm is 8 years old and still beeps when I press test. Is it OK?
Probably not. The test button verifies the sounder, not the sensor. UL 2034 listed CO alarms have end-of-life sensor chemistry; manufacturers specify 5-7 years for older RV units and up to 10 years for newer combo. The unit may sound when CO is present but only at higher levels than spec. Replace at the date stamp.
What is the difference between a CO alarm and a CO detector?
Loose terminology, often used interchangeably. A CO alarm is the consumer-grade UL 2034 listed unit that sounds when CO exceeds threshold. A CO detector / monitor typically refers to a calibrated direct-read instrument that shows ppm — used by fire departments and HVAC technicians. Both serve different purposes; the alarm is mandatory in RVs, the meter is useful for spot-checking.
